Tokyo Underworld (深東京, Shin Tōkyō?) is the 1st chapter of Tokyo Underworld manga, written and illustrated by Kenji Sakaki. This also serves as the first chapter of the first volume of the manga series.

Yomi Kanda, a 16-year-old student, was observed defending their homeroom teacher, Masatoshi Yabuuchi, who is 60 years old, against a classmate known as Kashiwagi, who has a reputation for being a troublemaker. Yomi intervened when Kashiwagi nearly pushed their teacher through a classroom window but ended up getting hit by Kashiwagi in the process. Yomi's twin brother, Yami Kanda, arrived on the scene and pulled Kashiwagi's hair to prevent him from harming the teacher further. Both Yomi and Yami had visible injuries on their faces, leading Kashiwagi's friends to speculate that they had been in a fight with others.
Yami threatened to harm anyone who attacked his twin brother and briefly pushed Kashiwagi towards the classroom window. Masatoshi Yabuuchi, the teacher, mentioned that this kind of behavior was not uncommon in their class, Class 2-A, which prompted the school's dean to grant the brothers some leniency based on the teacher's statement.
Outside the office, Yami expressed his anger at Yomi for getting involved in other people's conflicts, putting himself in potential danger. Yami reminded Yomi about the phenomenon known as Twin Synchronicity, where one twin feels the same pain as the other when severely injured. Yami advised Yomi to focus on himself, emphasizing that their teacher deserved support from his own family, just as he had protected and helped them against their teacher's bully.

The teacher, seemingly tired of further conflicts, bid farewell to the twins and suggested they prepare for an upcoming field trip. As the twins walked away, the teacher appeared to be reading tweets on his phone related to a triangle-shaped drawing.
Later, Yomi, who had finished eating, found Yami smoking by his bedroom window. Yomi scolded his brother, warning him that their parents would be furious if they were present. This conversation led them to reminisce about their past.
Their parents had disappeared five years ago, and the police had given up the search at the time. The brothers had undertaken an extensive search, visiting 23 towns across Tokyo in an attempt to find their missing parents. Yami believed their parents were alive somewhere, relieved that they had left their troubles behind. Yomi disagreed, believing there must be a reason for their parents' absence. Yomi recalled comforting his crying brother at the time, promising that they would work together and be fine as long as they had each other. With those words, Yomi left his brother's room, turning off the lights.
It was announced that they would go on a field trip to Tokyo Tower, but Yami was unimpressed as he found it uninteresting. As they argued on their way to the tower, Yomi noticed Haine Saikawa looking out the window at the tower's view. Yomi approached her and complimented the view, but she responded coldly. Yami complained about his brother, and Haine mentioned she despised Tokyo Tower and brought up something called "Tokyo Underworld," which confused Yami and Yomi since they hadn't heard of it.
Haine explained that it's another Tokyo where those who have caused someone's death are punished. Yami remembered that it was thought to be a real-life hell with sinners and demon guards. Haine mentioned social media claims of people escaping from there, treating it as an urban legend.

Masatoshi drank from the tumbler, which contained coffee laced with pesticides, and accused the students of driving him to his death.
Filled with rage, he threatened to kill them all and ensure their punishment, then suddenly collapsed, bleeding from his mouth. The bus began to tilt, causing panic among the students, and the sudden death of their teacher added to their distress.

The students were horrified, both by their teacher's death and the chaos that ensued. As they descended, Yomi noticed that they weren't alone; they were under observation. Haine woke him from his trance, and he was immediately surprised to find his unconscious brother, Yami, beside him. When the three of them finally stood up inside the bus, they were shocked by the situation. All their classmates were unconscious on the bus floor, and they gradually woke everyone up. Haine checked the bus driver's seat, but the driver was missing, and her phone had no network signal.
As they turned around, they saw their teacher's lifeless body, which was a tragic outcome after he had vomited blood earlier. They felt remorse for what had transpired, culminating in the teacher's suicide. Most of them blamed Kashiwagi, who had been the main instigator of the bullying against the teacher. In response, Kashiwagi argued that they were all equally responsible for not intervening to help the teacher, claiming they had no right to accuse him. The brothers were infuriated by Kashiwagi's words and advised him to stop talking to avoid further problems.
Yomi took the lead in planning their next steps, suggesting they stay calm, exit the bus, and seek help. After a while, one of their classmates noticed that the bus was moving, causing everyone to panic and rush out.
Once outside, Haine became overwhelmed by fear when she spotted what seemed to be a large eye-like camera positioned at the center of the tower. This camera was only visible from the bridge they were on. In response to her fear, she said, "No, I can't. Not Tokyo Tower!" while hugging herself tightly.
